New members must bring birth dates for all dependents and a $25.00 Enrollment Fee at time of enrollment.
The Plan year is November 1, 2014 - October 31, 2015.
Weekly payroll deductions will be over 30 weeks, beginning in October 2014 and ending in May 2015.
